Surgery Report Procedure: Transpupillary barrier retinopexy Began anesthesia at 12:10 Began procedure at 12:30 Ended Procedure at 1:30 Ended anesthesia at 1:40 Eyes were lubricated with optixcare eye lube. 035 was pre-medicated with midazolam at 1mg/kg and induced and maintained on isoflurane. Surgery report |
Procedure - Mass removal | Performed by - Sokoloff | Surgery time - 50 minutes | 035 was induced and maintained on isoflurane. Heart rate was
monitored via doppler and respirations were monitored by movement
of the keel. Heat was provided to the patient by a heating pad
placed beneath a towel. Breaths were intermittently provided
during the procedure until the last 15 min. which then they were
given 20 times a minute.
The skin was incised superficial to the mass in a circular shape
about 1.5cm in diameter. The mass was dissected from healthy
tissue. It was observed that the mass invaded the wall of the
cloaca and could not be dissected away from the cloacal wall.
The mass was excised from the cloacal wall. After excision, the
surgery site was lavaged, and the cloacal wall was closed by
a simple continuous suture pattern w/ 5-0 PDS. The tissue of
the cloacal wall was then inverted with a Cushing suture pattern
also with 5-0 PDS. A splash block of 1.9mg of 20% lidocaine diluted
with LRS was performed at the surgery site after closure of the
cloaca. The outer skin layer was then closed with simple interrupted
sutures and covered by a tegaderm pad. 0.38ml (0.1mg/ml) of flumazenil
was administered IM into the right pectoral muscle. Recovery
from anesthesia was smooth and no complications were encountered.

Radiograph Report: The coelomic cavity had gas opacities between the heart and liver indicative of a pneumocoelem. An object of soft tissue opacity and speckles of mineral opacities was observed in the caudo-ventral portion of the body, just cranial to the vent.
Ultrasound: The mass cranial to the vent was the area of interest during ultrasound. There was anechoic substances within the mass indicating it had some fluid. The mass also displayed some shadowing, probably from more firm tissues within the mass. The doppler mode on the ultrasound machine revealed blood flow in the superficial layer of the mass.
Fine Needle Aspirate: A cloudy yellowish fluid with a pungent odor was aspirated from the mass. When stained and viewed under a microscope, there was a copious amount of rod bacteria

EPH Interpretation The beta globulins are increased and reflective of acute inflammation. Moderate to marked increases in this fraction can be observed with nephritis, hepatitis, dermatitis, and mycotic infection. Increases may also be observed with sarcocystosis and chronic chlamydiosis as well as mycobacteriosis. Specific beta globulin proteins are also increased in egg laying birds. Followup EPH may have prognostic value in this case.

Assessment: 035 is a Adult Red Tail Hawk of unknown sex that presented to the Southeastern Raptor Center for a cloacal mass. On Physical exam 035 was observed to have a poor body condition (1.5/5), dehydrated, a cloacal mass and pigment of the vitreous of the left eye. The ocular abnormalities and body condition are likely a result of a traumatic event.
Plan: 53ml LRS SQ BID Ketorolac OS BID Meloxicam: 0.18ml (5mg/ml) IM BID TMS: 0.94ml (48mg/ml) PO BID Wait for HPAI test results to come back.
